Position Available Case Manager Assess post-hospital needs of patients, families and significant others and facilitates plans appropriate to the patients needs. Evaluates hospital admission, treatment and length of stay to meet patient care needs, coordinates the patient care needs and to comply with the Methodist Hospital Utilization plan. Coordinates the communication process and initiates referrals to facilitate the patient transition through the system. Position Requirements: Bachelors Degree (B.A.) from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Certification for Case Management preferred. Licensed as a Registered Nurse in the State of Pennsylvania. 2-3 years as an acute care staff nurse. Full Time, Monday - Friday, day shift with some weekend req.
