Form W-9, Request for Taxpayer Identification Number
and Certification
Form W-9, Request for Taxpayer Identification Number
and Certification, is an Internal Revenue Service (IRS) form that
is used to provide taxpayer identification numbers (TIN) to the Accounts
Payable department. For individuals, TIN is their Social Security number.
For other unincorporated entities, such as sole proprietorships or
partnerships, it is their employer identification number (EIN). The
taxpayer identification number is required by Accounts Payable for the
proper filing of information returns (1099) with the IRS to report income
paid to any unincorporated entity. A copy of the form is available
through Accounts Payable (X3-6475).
Requests for Payment to independent contractors
cannot be processed unless Form W-9 is attached to the
initial request. Refer to TJU University Policy #103.19 for more
information.
