Jefferson School of Population Health (JCPH) and Jefferson College of Graduate Studies (JCGS) Financial Aid Application Grid.
This document identifies all documents that must be completed to apply for financial aid from either JCHP or CHS during the 2001-2002 year.

Additional Notes About Applying for Financial Aid from Thomas Jefferson University
- STUDENTS WHO COMPLETE THE FAFSA RENEWAL FORM OR THE FAFSA ON THE WEB are not required to complete the blank FAFSA included in the JCHP/JCGS Financial Aid application packet. If you complete the FAFSA renewal form or the FAFSA on the web, please return the unused FAFSA to the University Office of Student Financial Aid for recycling. Please be sure to use the correct school code for the college you will be enrolled in during the 2003-2004 academic year. JCHP code - 013549 JCGS code - E00799 (Please note: All third year Physical Therapy students use JCGS code E00799 when completing the FAFSA.)
- STUDENTS WHO WISH TO APPLY FOR FEDERAL WORK STUDY (FWS) should complete the Federal Work Study Job Application (included in the application packet) in addition to completing the application requirements outlined on the reverse side. Please note that completing the FWS forms does not guarantee that the student will be awarded federal work study funds.
- STUDENTS WHO ARE PERMANENT RESIDENTS must submit a photocopy of both sides of their I-151 (green) card to the University Office of Student Financial Aid.
- STUDENTS WHO ARE APPLYING FOR INSTITUTIONAL AID are also required to apply for the maximum Federal Subsidized Stafford loan. Please complete the Federal Direct Stafford Loan section of the institutional application and, if you have a previous lender outside of Pennsylvania or have never borrowed a Federal Stafford loan before, please contact the University Office of Student Financial Aid to obtain a Master Promissory Note.
- NEW STUDENTS WHO BORROW OR PLAN TO BORROW through Institutional loan programs, the Federal Subsidized Stafford loan program, and/or alternative loan programs MUST attend an Entrance Interview. Please note that Federal Stafford and institutional loan funds may not be disbursed until the Entrance Interview requirement has been completed. Please contact the financial aid office for more information regarding this requirement.
