History

Pt. is a 44 y/o female pmh. HTN, NIDDM w/ several month h/o chronic intermittent diarrhea. Frequency and amount of diarrhea is variable: 1-5 x /day. Sometimes solid.

She denies BRBPR, melena, abd. pain, N/V, or weight loss. No antbc use in last 6 months. No recent travel history.

In addition to c/o of diarrhea she reports a soft, non-tender 'lump' in her epigastrim. She states that she noticed it over a year ago.

PMH: HTN, NIDDM, h/o nephrolithiasis.

PSH: Choley, tubal ligation, tonsillectomy, breast cyst excision.

MEDS: Metoformin, lansoprazole, fenofibrate, fluoxetine, HCTZ, glipizide.

ALL: IVP dye

SOCIAL HX: (+) Tobacco. Social EtOH IVDA. Chemist.

FAMILY HX: M lung cancer; Paternal grandfather -> colon CA; Maternal grandfather -> prostate CA
