AOTA’s Fieldwork Performance Evaluation (FWPE): this is the tool you’ll use to evaluate students and midterm and final. At midterm, the students should also evaluate themselves with this tool and the perceptions of each can be used as a springboard for discussion

TJU Student Evaluation Of Fieldwork Experience (SEFWE): this tool is completed by the student at the end of the placement, and the feedback is used by the site/supervisor to re-evaluate and/or refine the FWII program

AOTA’s Self-Assessment Tool for Fieldwork Educator Competency (2009) : supervisors can use this tool to identify areas of strength and need related to their skills in fieldwork supervision

Developing Site Specific Learning Objectives: This PowerPoint presentation has some good suggestions to help you develop your site specific learning objectives, which are required for every level II placement. Additional information and examples can be found in the fieldwork educator section of AOTA’s website.

The Fieldwork Education Assessment Tool (FEAT): This tool from AOTA should be completed at week 4, by both the student and the supervisor, to provide formative feedback about how the experience is going so far, and areas that can be worked on

Weekly Supervision Form: This form should be used weekly in order to provide the student with both positive and constructive feedback, and to clarify goals for the future
