Project Title: Extending Scarce
Resources: Utilizing eHealth Tools, Universal Strategies, and
Campus-Community Partnerships to Promote Comprehensive Health
and Wellness within a Rural, Underserved American Indian Community.
Target Population: The Omaha and Winnebago Ho-Chunk
Indian Tribes are located on reservations in the northeastern
corner of Nebraska in Thurston county, a rural county. American
Indians account for approximately 52% of Thurston County's population.
Project Overview: The Creighton University Team plans
to pilot this program by interfacing with the Tribal Community
Health Education Office and Diabetes Program to identify and
train three community members to take on the role of CHAs, bringing
health information to the community. Training sessions on the
basics of computer and Internet usage will be implemented first
to minimize isolation faced by rural American Indians and to
increase access to allied health resources. A Website will be
developed for the CHAs. The Website will include: a list of links
to American Indian/Native American health related Websites, focusing
on the demonstrated health needs of the Omaha Tribe of Nebraska;
training modules; and resources on how to evaluate health information.

Project Title: A community-based
eHealth program to provide practical information, guidance and
support to senior citizens and caregivers, and the businesses
that work with them in Elko County.
Target Population: The population is composed of 72.4%
White non-Hispanic, 5.3% American Indian/Alaska Native, 19.7%
Hispanic/Latino, and 2.6% other.
Project Overview: To develop an eHealth infrastructure
for senior citizens and caregivers that will include a health
promotion website and computer/Internet training at senior-accessible
computer sites. The website will include: 1) video footage teaching “how
to” strategies and techniques, 2) education modules that teach
senior citizens and their caregivers different strategies that
will increase or maintain their functional abilities, and 3)
an interactive program whereby caregivers can perform simple
assessments, enter the information into the Internet program
and be provided with specific strategies or advice to enhance
the function and safety of the caregivers. |

Project Title: Care of the Frail
Elderly
Target Population: Frail elderly
Project Overview: To develop a website specifically
for the care of the frail elderly. can be This website will include:
1) the latest research concerning each of the four syndromes,
2) treatment protocols for the four syndromes designed by Holy
Redeemer, and 3) true stories of patients helped by Holy Redeemer's
program. Community outreach is an important part of a comprehensive
health program, and an area we look forward to developing further. |

Project Title: Talking Fotonovelas
for Low Literacy Hispanic and African American Populations.
Target Population: Hispanics and African-Americans with
low literacy levels in English and/or Spanish.
Project Overview: The program proposes to establish
a Web-based site containing talking fotonovelas to be
accessed by different service delivery sites. The Web-based culturally
and linguistically appropriate talking fotonovelas will be designed
to complement Speech and Language Pathology, and Audiology services.
They will offer low literacy clients the opportunity to receive
clinical instructions and health information in a format that
will increase their understanding of: a) service procedures,
b) the nature of speech and language pathologies and hearing
problems, and c) clinical recommendations. |

Project Title: Healthline
Target Population: Springfield's residents. (Black, non-Hispanic,
21%; Hispanic/Latino, 27%; Asian, 4%; Caucasian, 48%)
Project Overview: To expand the existing STCC
School of Health Website, http://health.stcc.edu, which presently
provides a variety of health career resources and health information.
Because the infrastructure is already in place, the proposed
website can be built upon with the goal of having it serve as
a user-friendly, single point resource of health information.
The Community Health Advocates will use this resource to: 1)
link individuals with accurate available health information,
2) serve as “gatekeepers” of the Website's information, and 3)
guide and direct as to what those needs are. |

Project Title: Promoting Home Safety
for Homebound Individuals
Target Population: Individuals ranging in age from young
adults to the elderly who are within their home as a result of
illness, disability, or isolation/lack of social network. The
racial and ethnic composition of these individuals includes African
American, Caucasian with strong ethnic European ties (polish),
and individuals of Hispanic descent.
Project Overview: The purpose of this program is to
decrease the risk of injuries and/or health deterioration as
a result of home safety issues. This will be accomplished by
creating a website that provides simple home modification suggestions
to enhance safety, and links to community networks. Therapists
and students will provide in-home health education with the aid
of wireless laptop computers. Clients will receive demonstration
on how to access the Internet and will be educated on the information
that can be found there. |

Project Title: Community Caregiver
Project.
Target Population: Caregivers in the east Los Angeles
area. 89% are Hispanic, many of whom speak and understand limited
English.
Project Overview: The aim of the program is to develop
a website to meet the needs of the caregivers residing in the
east Los Angeles area. Pages will focus on individual topics,
allowing people to review the material they are most interested
in. Planned topics include the following: normal ageing, performing
range of motion and preventing/managing contractures, keeping
skin healthy while preventing ulcerations, healthy diets, communication,
nutrition, swallowing concerns, assisting with mobility, depression,
working with your physician, diabetes management, continence
management, and coping skills for the caregiver. Links to sites
with accurate information will be provided for each topic, as
able. |
