Application Guidelines

Each team should submit the following materials to be considered for participation in the eHealth Training Institute. Applications should be clear and concise.

  1. Application Form (56.4 KB Adobe pdf file)
    This file requires the free Adobe Acrobat Reader.

  2. A proposal describing your current work and rationale for attending the Institute. This proposal should be a maximum of five double-spaced pages (approximately 1,000 words) containing the following information:

    Description of underserved population you are currently working with or plan to work with
    A description of the population containing a documented demographic profile which should include; number of individuals, racial and ethnic composition, age, estimated literacy level, major health needs, and any other descriptive information that would help describe this community. In addition, a description of your relationship with the community should be included. This might include how long you have worked in the community and a description of any on-going or past projects if applicable.

    Description of health program you are planning to conduct or are conducting
    A brief description of a health program (overview and major components) you would like to conduct or are currently conducting in this community.

    A statement regarding how your program will make a difference in this community and plans for how you would disseminate the success of your program to others
    A statement providing your vision of the potential outcomes of the project as a result of attending the Summer Institute. This statement should also contain a plan for dissemination of information regarding the impact of the program on the community.

    Statement of what your team expect to gain from the Institute
    Provide a statement describing the anticipated responsibilities of each member of your team on the proposed project.

  3. Two letters of support.
    One from your department chair or clinical supervisor providing a commitment for your participation in the Institute and a second from a community leader with agreeing to work with you on the project.

  4. A biosketch of each team member, not to exceed two pages.

General Guidelines for Proposal Evaluation

Each proposal will be evaluated according to the following criteria:

  1. Value of program plan to each profession
  2. Thoroughness of proposed eHealth program plan
  3. Demonstrated strength of institutional support and community partnership
  4. Expression of vision and next steps
  5. Potential for future contribution to the health of underserved individuals
