Student Health Insurance
Annual Matriculation Requirement
Note for New Students:
New students will receive an Orientation packet in the mail from the Office of Student Affairs approximately one month prior to their scheduled Orientation date. The Orientation packet will contain information about the health insurance requirement. The 2008-09 website for health insurance is anticipated to be available in mid-July. New students should not attempt to complete the requirement prior to receiving their packet.
***IMPORTANT*** All matriculated students in the Jefferson College of Health Professions must maintain health insurance and must complete an ANNUAL enrollment/waiver process. The enrollment/waiver process is for each academic year, beginning September 1st and ending August 31st of the following calendar year.
Please note the following procedures for enrolling/waiving health insurance coverage for the 2008-2009 academic year. All enrollments/waivers must be completed using the online process outlined below.
As you prepare for the coming semester, health insurance may not be high on your list of concerns. Nevertheless, adequate coverage is essential, and we ask that you review your coverage at this time. You may be covered by the policy of your employer, spouse or parents but if not, you may consider the school-sponsored group health insurance plan that is available to all matriculated students of the Jefferson College of Health Professions.
As stated above, Jefferson College of Health Professions requires all matriculated students to have health insurance. You will be expected to complete the enrollment/waiver process as outlined below. Failure to complete the process by the first day of the fall semester (September 3, 2008), will result in a student's automatic enrollment in the college-sponsored Student Health Insurance Plan. The premium for the Plan will be added to your tuition bill. Also. please note that if you do wish to enroll, you must complete the online process - do NOT fail to take action and assume that you will be automatically enrolled - doing so may result in a delay in benefits and inaccurate subscriber information.
Once enrolled, the plan will be in effect for the entire academic year. Students may only terminate their insurance coverage if they have a qualifying life change event. You may visit the Plan’s website for additional information on life change events.

Minimum Health Insurance Requirements
In order to qualify for a waiver, your insurance must meet at least the following minimum requirements.
- My plan provides in-patient care in the Philadelphia area.*
- My plan provides out-patient care in the Philadelphia area. *Coverage for
emergency-only care does not satisfy this requirement.
- My plan provides for ancillary procedures, including laboratory tests and
radiology.
- My plan provides for outpatient and in-patient mental health care in the
Philadelphia area.*
- My plan includes prescription drug coverage.
- This coverage will remain in force through August 31, 2008.**
* For students attending the Geisinger campus, coverage must be in effect for the Danville area (in place of the Philadelphia area). For students participating in distance learning, coverage must be in effect for your area of permanent residence.
** If your insurance will not be in effect for the entire academic year due to a life change event, you may qualify for a partial waiver during the period that your insurance will cover, and will then have to enroll in the school-sponsored insurance plan for the remainder of the academic year.

Enrollment/Waiver Process
THE ONLINE SYSTEM FOR 2008-09 IS ANTICIPATED TO BE AVAILABLE IN MID-JULY. INSTRUCTIONS ON HOW TO COMPLETE THE ENROLLMENT/WAIVER PROCESS WILL BE POSTED HERE PRIOR TO THAT TIME. PLEASE CHECK BACK FOR UPDATES.
NOTE:
F.A.C.T. Students
Students that are enrolled in the F.A.C.T. Nursing program will need to complete the insurance requirement for two academic years. When students begin their program in the summer, they will need to complete a hardcopy health insurance waiver/enrollment form. This form will be mailed to students in their Orientation packet, and will be in effect for the months of June, July, and August. F.A.C.T. Nursing students will later need to complete the health insurance waiver/enrollment process for the new academic year, which starts September 1st. Students should follow the instructions above to complete the process online for the new academic year.
Incoming Students for January
Students that will be starting their academic program in the spring semester (January) will need to complete a hardcopy health insurance waiver/enrollment form. This form will be mailed to students in their Orientation packet, and will be in effect for the months of January through August. Students will then need to complete the health insurance requirement for the new academic year, which starts September 1st. Students should follow the instructions above to complete the process online for the new academic year.

Dental Insurance 2008-2009 Information
Optional dental insurance is available to JCHP students, the coverage is offered through Denex Dental. This plan is a custom PPO and provides full coverage of preventive services (exams, cleanings, x-rays) and discounted rates on most other basic dental services, after your deductible.
Rates for the 2008-2009 year:
Student: $191.52
Student + 1 or more dependents: $662.64
