CHECK YOUR CHECK FOR W-2 INFORMATION ------------------------------------ The payroll office will soon be preparing W-2 Forms to mail to all Jeffersonians by January 31, 1995. A W-2 Form is the individualized statement, issued by an employer, that shows gross taxable earnings for the preceding calendar year. Copies of this form must be filed with Federal and State Income Tax Returns. To ensure that the information contained on your 1994 W-2 Form is accurate, please check your next paycheck stub and departmental records to verify that your name, address and social security number are recorded correctly. If any of these items are incorrect, notify your timekeeper who can make the necessary changes for you. If you have received any disability payments from the University's insurance companies during 1994, you will receive a separate, Third-Party Sick Pay W-2 Form showing the total of disability payments. Please note that the amount reflected on that separate W-2 Form must be included in the total earnings amount you claim on your Federal and State Income Tax Returns. DON'T MISS THE WOMEN'S BOARD JEWELRY SALE ----------------------------------------- Come to the Gibbon Building's 11th Street Atrium on Tuesday and Wednesday, December 13 and 14, between 9 a.m. and 3 p.m. for the best possible prices and a wide selection of jewelry direct from the manufacturer, as well as imports from Italy, Israel and Asia. JEFFERSON CAN FILL IT ---------------------------------------- Do you know someone who's retired or a parent who has free time during the day while the kids are in school? Young, not so young or somewhere in between, there's a volunteer opportunity awaiting them at Jefferson. Volunteers who can spare several hours at a time during the day, Monday through Friday, are especially needed. Volunteers can choose from a variety of jobs in a variety of offices and outpatient areas. Whatever they do, volunteers serve as Jefferson ambassadors and assist us in providing the quality care patients expect from Jefferson. If someone you know wants to make a contribution to Jefferson and the people we serve, call the volunteer department at 6222.
