New Forms Coming Your Way July 1
--------------------------------

   Four forms frequently used by Jeffersonians will be revised and replaced
effective July 1. Beginning then, please be sure to use the new forms,
which will indicate "REV.7/94" next to the form number on the following
documents:

   o Request for Purchase (Form 0027-00)
   o Report of Travel Expenses (Form 0013-00, formerly called Report of 
     Individual Expenses)
   o Request for Payment (Form 0338-01)
   o Inter-Departmental Charges (Form 0326-00)

   Details on obtaining new forms and discarding old forms will be passed
on to department heads and key personnel over the next few weeks. In
addition, they will also receive information on a training program,
scheduled for early June, which has been designed to help employees work
with the new forms and related policies. Exact dates, times and locations
will be provided.
