Maintaining the accuracy and integrity of hospital documents and records is a responsibility we all share. Our billing and claims processing systems rely on the accuracy of the medical record, including the services and procedures documented in the record to bill Medicare as well as all other insurance companies for services to patients. In fact, one can argue that if a service was not documented, it didn't happen.

To ensure the integrity of our medical records, please document the service as soon as possible after the time when the service was rendered. Late entries arouse suspicion and also increase the likelihood of inaccuracies.
