We offer complete services for surveys and evaluations including instrument design, data collection, statistical analysis and reporting.
Data can be collected on the web or on paper using optical character recognition technology that reads handprint as well as “bubble” marks. Custom design is available for program evaluations, patient surveys, rating forms, tests, and medical record abstracts.
The two most important factors affecting cost are complexity and the volume processed.
A typical one-page paper form with standard layout and limited handprint fields, barcodes and comment boxes can be developed for as little as $300, which includes processing the first 100 copies. A comparable web-based instrument will be more expensive to set up, about $500. However, the costs for additional respondents are minimal.
The cost for a complex one-page form involving custom layouts, atypical response scales, or multiple handprint fields ranges from $500 to $900. Complex instruments suitable for web administration begin at $700.
Standard reports include simple descriptive statistics. Raw data are available in a variety of formats including Excel, Stata, SAS, or SPSS.
