The Jefferson Medical Home Academic Center (Jeff MHAC) in the Department of Family and Community Medicine is the Center for Patient Centered Medical Home (PCMH) Research, Training and Dissemination, funded through HRSA. PCMH is a distinctive relationship based program that aims to engage both physicians and patients in an effort to strengthen the doctor-patient relationship and better coordinate care to achieve ONE care delivery system. Jefferson Family Medicine Associates (JFMA) has been recognized with the highest ranking from the National Committee for Quality Assurance (NCQA) as a Level 3 PCMH.

Jeff MHAC Goals:
- To improve health care access to vulnerable populations through enhancement of clinical teaching and research programs. This will nourish the academics by increasing the number, quality, distribution and diversity of primary care physicians taking treasure for the underserved populations.
- To generate evidence based tools for training and dissemination of the concept of patient centered care in an academic setting.
- To integrate primary care with improving population health outcomes.
