support of Jefferson!
Plates are available for a one-time fee of $75, which includes a $20 state fee plus $4 for pressing the plate; the remaining $51 is a tax-deductible contribution to Thomas Jefferson University.
To order:
- Print the application form. If you do not have access to a printer, please e-mail your contact information, including your full name and mailing address, to melissa.chirinos@jefferson.edu or call her at (215) 503-7709 and an application will be mailed to you.
Please note that the form lists the fee as $20 and not the full amount, $75.
- Complete the application, retain a copy for your records, and mail the original along with your top three choices for numbers 1 through 100 and a $75 check made payable Thomas Jefferson University to:
