Protocol Forms

The Institutional Animal Care and Use Committee must approve all research protocols in advance of project undertaking. The IACUC meets monthly, every 4th Tuesday of the month. The deadline for protocol submission for these meetings is the first Friday of each month, by 12:00 noon. All new protocols and protocols requiring a triennial review must be pre-reviewed by a committee member, prior to submission to the IACUC.

Please note: All 3yrs. renewals must be pre-reviewed by an IACUC member.

Timely submission of the following forms in electronic format, as instructed on each form, satisfies requirements.

Protocol submission through the IACUC Jeffshare has been instituted to so that the IACUC Office can provide better service to our Research Community. Please keep an eye out for your invitation to the share in your email inbox. To provide the best service possible to our customers, Jeffshare submission will eventually be the only means to submit materials to the IACUC. Please feel free to contact the IACUC Office with any questions or concerns.

Submit a separate Form RO-5 for each animal use procedure and species.

As a service to our Investigators and their Support Staff, the IACUC now offers Standard Operating Procedures for some commonly used procedures, such as blood collective and imaging. These IACUC approved SOPs may be referenced in the protocol in place of descriptions for these procedures. If a protocol includes a reference to an IACUC approved SOP in place of a procedural description, then the SOP must be rigidly followed just as if it was part of the protocol.
