Annual Influenza Vaccination

University Health Services began the annual influenza vaccination program earlier than in past years, highly encouraging vaccination to all employees and students of TJUH and TJU.  We may have sporadic supplies of seasonal flu vaccine available as we approach the normal flu season.   Please feel free to call to confirm availability prior to coming to UHS for your vaccine. Vaccination against the seasonal flu continues to be a prudent step to take to protect yourself, your patients and your family.  All hospital employees are required to undergo vaccination OR sign a declination form.  Vaccination is strongly encouraged. 

The consent/declination as well as the Vaccine Information Sheet (VIS) are available on this site. We ask that you read through the information on the consent/declination and complete the form in its entirety.  If you receive the vaccine at another location, a declination must be signed with the date and location of vaccination.  UHS staff is available to answer your questions at 5-6835 regarding the influenza vaccination or the forms.

                                                        H1N1 (Swine Flu) Update

The Philadelphia Department of Health is working with health care facilities within the City of Philadelphia to distribute the H1N1 vaccine.  The annual influenza vaccine does not provide protection against H1N1 Influenza.

H1N1 vaccine is currently available at UHS for ALL employees and students who wish to receive it.

The following recommendations are for all employees and students:

  1. If you develop flu-like symptoms of fever, cough, sore throat, and body aches,do not report to work.  Contact your supervisor immediately.  If you are a student, we recommend that you contact UHS to discuss your symptoms.
  2. If you have been sick with flu-like symptoms of fever, cough, sore throat and body aches, you must be cleared through UHS prior to your return to work.
  3. Most cases of H1N1 Flu respond well to over the counter medications to treat the body aches and fever along with a light diet and rest.
  4. To prevent infection wash your hands frequently, cover your nose and mouth with a tissure when you cough or sneeze and avoid touching your eyes, nose and mouth.

PPD Placement

If you are a nurse who has been trained to place and read tuberculosis skin tests, the PPD placement form may be accessed through this site. PPD documentation form.  Please complete it legibly and return it to our office at 833 Chestnut Street, Suite 205 once the reading has been documented.  If you have concerns regarding the reading, please call us. Employees who test positive should fill out the form Reviewing Signs and Symptoms of Active TB. TB Annual review of symptoms.

Medical Evaluation for Respirator Fit Testing

Employees undergoing fit testing for the N95 respirator must complete the   medical evaluation form prior to the fit test.  The medical evaluation form is completed by the employee and forwarded to UHS for approval.  Approval forms are returned to the department's fit tester.  Completed fit test forms are returned to UHS for inclusion in the employee health record.

Pre-matriculation Requirements for Jefferson Students 

All students are required to submit a pre-matriculation physical examination form to UHS prior to registration.  Our address is listed below.  Current Jefferson employees may have this done free of charge in UHS.  Please fill out the first two pages of pre-matriculation form and bring to our office.

Episodic Care for Students  

Students may be seen for episodic care visits on a walk in basis during the hours of 7:30 am – 11:00 am and 1:00 pm – 3:30 pm.  Vaccinations and tuberculosis skin tests are provided at any time during the day.

Work Related Illnesses

Ill employees may be seen to determine their ability to work.  University Health Services will refer all employees to their primary care provider for evaluation and treatment

Occupational Exposures for Employees and Students 

Any employee of student who has sustained a body fluid exposure should be seen as soon as possible in UHS, or in the Jefferson Emergency Department during off hours.  See Occupational Exposure to Body Fluid Section at top left of page.

Adacel (Tetanus, Diphtheria, Pertussis Vaccine)

Adacel has been approved since June, 2005 and is recommended by the Centers for Disease Control for all health care workers.  All new employees/students are required to undergo vaccination prior to employment/matriculation.  Current employees/students may come to UHS for free vaccination or for more information.
