  • When submitting your name change form, please provide documentation demonstrating the change.  Acceptable documentation includes marriage license, court decree, or a government issued ID.

The forms below can be used by students in the:

  • Graduate School of Biomedical Sciences
  • School of Health Professions
  • School of Nursing
  • School of Pharmacy
  • School of Population Health

Schedule Change Form (Drop/Add)

  • This form should not be used to make schedule changes for SKMC.  The appropriate Drop/Add form for SKMC students can be found at the Office of the Registrar.

  • Schedules can be edited on Banner during online registration.  Please check your academic calendar for online registration dates.
