Requirements for Students Accepted to Jefferson School of Health Professions

Congratulations on your acceptance to Jefferson School of Health Professions!

Before you start your classes, please review the requirements and information listed below. If you have any questions about completing these requirements, please call the Admissions Office at (215) 503-8890.

Please use this form as a checklist to keep track of your requirements as you complete them.

Please note that if you do not meet each requirement by the specified deadline, then you are subject to cancellation of your registration, loss of access to the Banner Web Student System and on-line Pulse course information, inability to begin and/or remain in class, and/or inability to begin clinicals.

1. Log on to Jefferson email and the Pulse Portal intranet

Pulse is the official University web portal, providing access to JeffMail, Banner and JEFFLINE (the library). For a tutorial on using Pulse, go here.

  • The campus key is your personal identifier that allows access to information resources on campus. To access your Campus Key go to: http://pulse.jefferson.edu/
    (Click on the “Campus Key Lookup” under “Links” on the left hand side of the Pulse homepage.)
  • JeffMail is the university’s official correspondence method and is the only formal method of electronic communications between TJU and students, faculty and staff. Please check your JeffMail e-mail account at least once per day. Learn more about JeffMail here.
  • Read the enclosed IS&T letter to incoming students for technical information you need to know.

Within one month of acceptance


 

2. Accept the offer of admission

  • Go to http://pulse.jefferson.edu/ after you have accessed your Campus Key and password (as outlined above) and log onto Pulse.
  • Click on the "Banner" tab in the upper left corner of the page.
  • Choose "Banner Web Login" and access with Campus Key and password.
  • Select the "Student" link.
  • Select the "Admissions Information" link.
  • Select the "Pay Accept Deposit" link.
  • Pay the $300 non-refundable enrollment deposit.

Within one month of acceptance


 

3. Complete the Prerequisite Credit Agreement Form, if applicable

  • Complete and sign the form, then scan and email it to the address provided at the bottom of your form.
  • The form can also be faxed to 215-503-7241.
  • Complete all of the prerequisite credits prior to enrollment.

4. Update your permanent mailing address

  • It is important that all students review and update their HOME and LOCAL ADDRESSES, and EMERGENCY CONTACT information on a regular basis!

    Log on to Banner Web click on the Personal Information link, and proceed from there.

5. Attend Orientation

6. Attend Welcome Day, if applicable (Fall start students)

This program is coordinated through the individual academic areas. Please contact your academic department or school for information. (This does not apply to part-time General Studies students.)

7. Submit Student Medical Records to University Health Services

All matriculating Health Professions students must complete and submit the pre-matriculation health requirements no later than 30 days prior to the first day of class. 

  • CONSEQUENCE OF NON-COMPLIANCE: no access to Blackboard/Banner on the first day of class.
  • Forms may be found at the University Health Services (UHS) website www.jeffersonhospital.org/uhs  on the Student Services tab and must be completed by the student’s physician.
  • Health Professions Students who are listed in Tier I and must follow the requirements listed for Tier I.
  • The completed forms are submitted electronically to American DataBank, a company contracted by University Health Services, to gather and store the student pre-matriculation records.
  • Students enroll with American DataBank for a $30 annual subscription fee and upload the completed health records into the American DataBank Complio System at https://jefferson.complio.com. The required health forms may also be accessed through this website.
  • American DataBank and University Health Services will review the forms and communicate with the students via the email selected by the student when opening the ADB account.  All Jefferson students are expected to use their Jefferson email account when enrolling in ADB.
  • Students will see communications from the staff regarding outstanding requirements or insufficient requirements.
  • It is the student’s responsibility to verify that the forms have been reviewed and approved.
  • The forms have been accepted and are considered complete if the ADB Complio System reads “School Approved”.
  • Begin this process early since it may take up to 8 weeks to complete.
  • Questions may be addressed by phone to UHS at (215) 955-6835 or by email to JeffUHS@jeffersonhospital.org.
  • If you are a current JEFFERSON EMPLOYEE, you must visit UHS and update your records.

 

One month before first day of class


 

8. Complete a Child Abuse Clearance

Begin this process as soon as possible, but no earlier than January 1 for fall term. January applicants should begin the process immediately.   

  • The Child Abuse Clearance must be completed and received by TJU no later than:
    • August 1 for the fall term
    • December 1 for the spring term
  • It may take up to EIGHT WEEKS for this process to be completed.
  • Be sure to check the box that says "Employment with a significant likelihood of regular contact with children".
  • Click here to access the Pennsylvania Child Abuse History Clearance form from the Pennsylvania Department of Public Welfare website.
  • When you receive the report, mail the original to:

9. Complete National Criminal Background Check

Begin this process as soon as possible, but no earlier than January 1 for fall terms. January applicants should begin the process immediately.

  • Click here to access instructions on how to request your Criminal Background Check from Verified Credentials.
  • The Criminal Background Check must be completed by the first day of class.
  • Do not mail the original criminal background check to TJU. Keep it for your records.
  • Verified Credentials will send an electronic notification to TJU that the check has been completed.

10. Complete Pennsylvania Criminal Background Check

Your Pennsylvania criminal background check through PATCH must be completed, and the original form submitted to the Department of Occupational Therapy by the first day of class. It must be dated between July 1 and August 31. Instructions on how to complete this process were previously provided to you by the Admissions Office and can be found here:  https://epatch.state.pa.us/Home.jsp . Failure to meet this requirement by the specified deadline will result in loss of access to the Banner Web Student System and on-line Pulse course information. You will not be able to attend class or fieldwork.

Please bring your report with you to Welcome Day and turn in to the OT Department.

15. Pennsylvania FBI Fingerprinting

Both Department of Welfare and Department Of Education

Visit https://www.pa.cogentid.com/index.htm to register and pay online for both FBI fingerprinting clearances. You will then choose a location to get fingerprinted. FEE: DPE $28.75 and DPW $27.50

You must complete TWO separate applications:

One for the Department of Public Welfare and one for the Department of Public Education

Please review the following information about the process:

1. Print out the fingerprinting cards from the FBI website

2. Register and pay online for both the DOE and DPW

3. Send TWO copies of your fingerprints to BOTH the DOE and DPW. On the back of two of the fingerprinting cards, put your registration number for the DOE. On the other two cards, list the registration number for the DPW.

*The information will all be uploaded into the Pennsylvania website, and the Dept of Education will have access to all the information within 48 hours. Within 7-10 days, both the DOE and DPW will you send you your clearance forms.

Please bring BOTH Department of Welfare and Department of Education Clearance Forms with you to Welcome Day and turn in to the OT Department.

16. CPR Certification

Students must have an active CPR certification throughout their time in the Occupational Therapy program at Jefferson.

The following criteria must be met:

  • American Heart Association Basic Life Support (BLS) for healthcare providers
  • Students must be covered for the entire course of the curriculum. Therefore, the two-year certification should not occur before July 1.                                                      

Thomas Jefferson University Hospital offers the required CPR course, however it is not necessary to take it at Jefferson. If you are interested in the course at Jefferson, you can enroll at the following link: http://www.jeffersonhospital.org/departments-and-services/jeffstat/education-center

Please bring your CPR certification with you to Welcome Day and turn into the OT Department.
