Your 2015 Open Enrollment period is now closed

Verification Period & Confirmation Statement

Verification Period: November 7-21

You may review your benefits summary on Employee Self-Service during the verification period (November 7-21). Once you have logged in, click on the Main Menu on top of the page. Click on Self-Service link, then Benefits, and then Benefits Summary. Enter 01/01/2015 and select GO to view your 2015 benefits.

Due to the significant amount of plan design changes, we will also be mailing home an Open Enrollment confirmation statement in mid-November. You will have until November 21 to verify your 2015 benefits and contact the HR Service Center (3-HRSC) if a correction is needed.

Benefits Plus – Enrollment Extended!
From November 7 – 21, you may also take advantage of this time to enroll in voluntary benefits through Benefits Plus. For more information, go to and login with your campus key and password. Enrollment will open again for Davis Vision care and MetLife Accident Insurance and Critical Illness Insurance.
