The Jefferson Medical Home Academic Center (Jeff MHAC) in the Department of Family and Community Medicine is the Patient Centered Medical Home (PCMH) Research, Training and Dissemination Center, funded through HRSA.

The Center Operations and Mission are guided by the following grant objectives:
To increase the capacity to carry out PCMH research and evaluation focusing on vulnerable populations, resulting in the completion of at least 12 projects during the 5 years.
To serve as a training resource to increase the capacity and ability to teach medical trainees to provide care through the PCMH, working collaboratively to transform at least 5 Jefferson FM community-based teaching practices serving vulnerable populations into PCMHs; and to assist additional Jefferson and other teaching practices serving vulnerable populations to adopt PCMH components and teach trainees to provide care through PCMHs.
To disseminate PCMH research and training outcomes, including at least 12 national or regional presentations and 6 peer-reviewed publications during 5 years.
