Certificates of Insurance/Contracts
Requests for certificates of insurance should be completed using the Request for Certificate of Insurance Form, found under the Forms section of the Risk Management web page. These requests are for approved off-campus events, evidence of coverage for clinical affiliations/agreements, and other TJU/JUP-related approved events/contracts.
For all new requests, a copy of the agreement/contract must be submitted in order for Risk Management to review the insurance requirements and be compliant with the agreement. If requesting an updated certificate of insurance, please reference your event and or submit a previous copy. All requests should be forwarded via the Certificate of Insurance request form found under the Forms section of the Risk Management web page, and forwarded to Antonina.Miller@jefferson.edu.
