Managing One’s Own Chronic Disease:  Does it Work?  What is the Evidence?

A seminar featuring:  Kate Lorig, RN, DrPH,

Director of the Stanford Patient Education Research Center

 

Dr. Lorig, creator of the internationally recognized Chronic Disease Self Management Program (CDSMP), will provide an overview of chronic disease prevention, treatment and management programs.  She will describe the history and development of the evidence-based CDSMP and its research, policy implications and program examples.

 

This seminar will feature a discussant panel including Jefferson Faculty (Dr. Molly A. Rose, Jefferson School of Nursing and Dr. Michael P. Rosenthal, Jefferson Medical College) and a community health leader (Lynn Fields Harris, Center in the Park) who will provide their insights on chronic disease self management.
