Physical Exam
Student Health Medical Record Requirements
All full-time students are required to submit a completed Student Health Record Form (PDF), which includes the student's medical history and a recent physical exam and documentation of all required immunizations and testing completed and signed by a health care professional. This Student Health Medical Record is also available at the Student Health Center.
New or re-enrolling students must submit this form prior to attending classes.
The University reserves the right to deny housing, to institute fines or to withhold registration for any student who does not meet the pre-matriculation requirements established by Student Health Services.
Can I get my physical exam at the Student Health Center?
No, the physical exam must be completed prior to your arrival to the University.
Where can I get a physical exam?
You may obtain a physical exam by:
- An outside healthcare professional in private practice
- An outside convenience or urgent care clinic
- A local public health department
