Report an Incident

Reporting a violation of the University Community Standards

Any individual with knowledge of an incident of misconduct involving a University Student or Student Organization can make a report to the Dean of Students Office. Reports of alleged policy violations by Students or Student Organizations can be submitted by completing the General Incident Reporting Form.

Reporting Discrimination

Any individual with knowledge of an incident involving Discrimination by a University community member can make a report using the Harassment (Discrimination) Reporting Form. Discrimination occurs when a person is treated adversely based on a characteristic protected by law including but not limited to a person’s sex, age, race, color, religion, sexual orientation, gender identity, gender expression, marital status, pregnancy, national origin, ancestry, citizenship, military status, veteran status, disability or any other protected group or status.
