Application Process

If you are interested in applying to the Sleep Medicine Fellowship Program, please complete and submit our application.  
Please ensure that you submit all of the following with your application:

  • Photograph attached to application
  • Copy of Curriculum Vitae (if your CV has any significant time gaps in your education, training or career experiences, please download, complete and submit a Gap Explanation Form)
  • Medical College Transcript
  • Dean’s letter
  • Three letters of recommendation; one of which from current program director
  • Personal statement
  • Copies of all unrestricted licenses
  • Statement from program director of years completed and number of months in each rotation in residency, including attestation of satisfactory completion of residency training.
  • Foreign medical graduates must provide evidence of a valid visa and a copy of the ECFMG certificate. Thomas Jefferson University Hospital accepts applications from J-1 and H-1 visa holders.

In addition, Applicants must provide one of the following:

  • USMLE Step 1, Step 2 and 3
  • National Boards Part I, Part II, and Part III
  • FLEX 1 and FLEX 2
  • COMLEX-USA Level 1, Level 2-CE, and Level 3

  • Otolaryngology
  • Pediatrics
  • Psychiatry
