If you are interested in applying to the Sleep Medicine Fellowship Program, please complete and submit our application (PDF).  

Please ensure that you submit all of the following with your application:

  • Photograph attached to application.
  • Copy of curriculum vitae (if your CV has any significant time gaps in your education, training, or career experiences, please download, complete, and submit a Gap Explanation Form).
  • Medical college transcript.
  • Dean’s letter.
  • Three letters of recommendation; one of which from current program director.
  • Personal statement.
  • Copies of all unrestricted licenses.
  • Statement from program director of years completed and number of months in each rotation in residency, including attestation of satisfactory completion of residency training.
  • Foreign medical graduates must provide evidence of a valid visa and a copy of the ECFMG certificate. Thomas Jefferson University Hospital accepts applications from J-1 and H-1 visa holders.

In addition, applicants must provide one of the following:

  • USMLE Step 1, Step 2, and Step 3.
  • National Boards Part I, Part II, and Part III.
  • FLEX 1 and FLEX 2.
  • COMLEX-USA Level 1, Level 2-CE, and Level 3.
