New Student Requirements

Student Health Medical Record Requirements

Attn: East Falls Full-time Undergraduate students, all International students and any students seeking care in the East Falls Student Health Clinic:

In order to assure compliance with the PA Health Dept and our university guidelines, you are REQUIRED to complete New Student Health Forms prior to your arrival on campus. Due dates: Spring semester is due by January 1st. Fall semester is due by July 15th.

Completion Instructions: Please go to the Student Health Services Patient Portal (https://jefferson.medicatconnect.com) and log in using your Thomas Jefferson University Email formatted as campuskey@students.jefferson.edu (use your actual campus key email). If you are prompted to select a university from the drop down, please make sure to select Thomas Jefferson University. Please follow all instructions on the portal — download the physical and TB screening for your Healthcare Provider and then can upload completed forms along with your immunization record, plug in the immunizations dates and upload your insurance card. Once reviewed, your record will show as “verified” meaning you have met all requirements. If anything is missing, you will receive an email in your campus key email with a link to a secure Medicat portal message explaining what is still needed.

Failure to comply with the new student health form requirements will result in a Dean’s Banner Hold on your account — impacting your ability to move onto campus or request a room change, register or drop/add classes, and acquire transcripts.
