Advanced Headache Diagnosis & Management Post-Graduate Certificate Program
To be considered complete and eligible for review, a TJAPP application must be complete with required transcripts, personal statement and all supporting documents (clinical license, resume, etc.) submitted in the directed manner and received by our office by the application deadline date.
Prerequisite Coursework/Other Requirements
- Applicants must have a clinical graduate degree as evident through professional licensure
- Applicants must have (or have had) clinical professional roles and be physicians (MD or DO), dentists (DDS), NPs, physician assistants, and PhD psychologists as evident via submission of a CV/Resume.
It is extremely important that you carefully read all directions listed below to ensure successful submission of your application materials.
Submit an application to TJAPP.
You can only apply for one program and one entry term. If you submit an application for more than one program or more than one entry term, only the first one will be processed. TJAPP will not issue a refund of the application fee(s) for additional applications.
Submit official transcripts from each undergraduate and graduate institution attended to TJAPP. Please note that transcripts are required regardless of number of credits taken or if the credits were transferred to another school. An admission decision will be delayed if you do not send transcripts from all colleges/universities attended.
If your coursework was completed outside of the United States, you must submit a course-by-course evaluation from one of the following international evaluation agencies:
Submit current active professional license(s) and certification(s) to TJAPP in the Program Materials section under "Documents".
Submit CV/Resume to TJAPP in the "Documents" section.
In 500 hundred words or less, tell us why you would like to enroll in this program and how you believe it will advance your professional goals. Submit to TJApp via the “Questions” section.
International students who are in the U.S. in F or J status are restricted to taking one online course per term per federal regulations.
