To submit an accommodation request, fill out the Accommodation Request Form through Accommodate and attach the required documentation. Below are the required forms for qualified, licensed healthcare provider(s) to fill out. Completed forms should be attached to a student's accommodation request submission. If submitted documentation is determined to be insufficient and/or incomplete, the student will be required to obtain updated documentation and resubmit the information to Student Accessibility Services with their request. 

Supplemental documentation, such as 504 Plans, IEPs, ICEPs, or documentation from a previous institution may also be uploaded with a student's accommodation request.
