Thomas Jefferson University requires that you have a completed health history form on file in the Office of Health Services by the time you register for classes.
Without a completed health history, you may not be permitted to register for classes. Additionally, the Director of Health Services may require you to undergo a complete physical and immunization procedure if you cannot produce the form on or before registration.
The health form must be completed by a physician. Please send the completed health form to the university as soon as possible so your registration will not be delayed.
Please go to the Student Health Services Patient Portal to upload required forms, enter immunization dates, and complete required forms. You will need to log in using your Thomas Jefferson University email formatted as CampusKey@students.jefferson.edu (use your actual campus key email). If you are prompted to select a university from the drop down, please make sure to select Thomas Jefferson University.
