Webinars for healthcare professionals about the Philadelphia Diabetes Prevention Collaborative & Referral Hub

These sessions will bring awareness to healthcare providers of the Greater Philadelphia region about the goals of the Jefferson Philadelphia Diabetes Prevention Collaborative, the recommendations on screening, testing and referring to National Diabetes Prevention Programs, how to refer patients to programs in the community and  the work of the collaborative to increase capacity, availability and sustainability of programs. 

The National DPP Lifestyle Change Program

The National DPP lifestyle change program is founded on the science of the Diabetes Prevention Program research study, and subsequent translation studies, which showed that making realistic behavior changes helped people with prediabetes lose 5% to 7% of their body weight and reduce their risk of developing type 2 diabetes by 58% (71% for people over 60 years old). - CDC

The Philadelphia Diabetes Prevention Collaborative is a regional multi-stakeholder effort, coordinated by the Jefferson College of Population Heath. This webinar is made possible by support from the American Medical Association, and in partnerwhip with Health Promotion Council, Centers for Disease Control and Prevention, HealthShare Exchange, the Philadelphia County Medical Society, the American Diabetes Association, and many more.
