Departments must complete and submit the following documents to email@example.com. The Office of International Affairs allows 1 month for observational medical experiences and 3 months for other observations, scientific and educational purposes.
- International Visitor Request Form (PDF fillable) for B-1 / Visa Waiver / H-4 / F-2, etc.
- Sample Invitation Letter
- No Patient Contact Certification
- Copy of Passport
The international visitor is required to have health insurance which includes evacuation/repatriation coverage for the duration of their activity at Jefferson. They may use health insurance from their home country (that covers their time in the U.S.), or purchase it online from one of these suggested providers:
Everyone coming to Jefferson must undergo a health screening in the Jefferson Occupational Health Network (OHN) for Employees & Students prior to the beginning of an assignment. The Volunteer/International Visitor Immunization Form must be completed prior to arrival for review at the OHN. For more information, visit OHN's website. Please note that any incomplete or inaccurate documentation will delay your clearance. If you have any questions about immunization clearance, contact firstname.lastname@example.org.
International Observer/Visitor Check-in at Jefferson
The international visitor must check-in at the Office of International Affairs before participating in any activity at Jefferson. They must bring the following documents to the check-in:
- Passport(s) and Visa(s)
- Any immigration documents
- Health insurance information
