Inpatient Co-management
Our inpatient team is a unique, inpatient co-management service that serves patients with complex substance use disorders and the medical sequala of their disease. The goal of the Jefferson Addiction Multidisciplinary Service (JAMS) is to evaluate and treat patients with substance use disorders in the inpatient setting by providing multidisciplinary, evidence-based, and non-stigmatizing care. JAMS can be consulted to evaluate patients at any time during their inpatient hospitalization and provide an assessment of their recovery goals. The team will assist with developing a specialized plan to treat their acute withdrawal syndrome as well as a longterm plan as the patient transitions back to the community. For all consults, we work collaboratively with the JAMS interdisciplinary team which includes specialized social workers, CRS (peer recovery specialist), pharmacy, community health workers, and a dedicated infectious disease team (ID silver). If eligible, the patients that JAMS sees will be evaluated and enrolled into our outpatient continuity service called the Bridge Program. The JAMS team also interfaces with Acute Pain Management Service (APMS) and the C/L Psychiatry service as needed.
Bridge Program
The Bridge Program is an innovative primary care integration model for people with severe substance use disorders and complex medical conditions. In this model, a single interdisciplinary care team provides comprehensive, coordinated, relationship-based care across medical outpatient, hospital, emergency department settings as well as addiction treatment system levels of care and recovery residences. In addition to the full spectrum of health needs, the program’s staff supports patients with social needs such as housing, legal aid, and food insecurity. The Stephen and Sandra Sheller Consult Bridge Program serves as a vital safe haven for patients in the earliest stages of medical and substance use recovery.
