Population Health Essentials provides a 3-day comprehensive overview of population health and outlines the fundamentals necessary to improve outcomes and reduce costs.

Participants with clinical leadership responsibilities may consider enrolling in the 2-day Population Health Management & Strategy Boot Camp which completes a 5-day Population Health Academy series. Attending five days can be applied to additional graduation education in the Population Health program at JCPH. 

The Population Health Essentials course enhances the understanding of how to practice population health  including care coordination, patient engagement, transparency, and accountability for all outcomes. Clinicians and healthcare leaders need to be able to work with and across stakeholder groups to coordinate the continuum of care,  align to economic incentives, and deliver the best possible care at the right time and at the best price. 

Through case studies and real-life workforce applications, participants will review and analyze key areas of population health.

Population Health Essentials Program Format

This virtual live 3-day educational program will encompass real-time  didactic sessions, readings, curated and ad-hoc discussions, and interactive live group exercises. Participants will be required to attend all sessions.

The Program Will Feature

  • Instructor led sessions featuring interactive discussions, case students and real-world workplace examples. 
  • Networking and learning opportunities with other participants and healthcare stakeholders facing similar challenges. 
  • Skills and knowledge to help focus your organization's population health strategy 
  • Online access to coursework and materials, during the program and up to 6 months after the program. 

During this Program You Will Learn To

  • Identify the key stakeholders in the US health care system and understand how and why their complex interrelationships influence population health outcomes.
  • Define and describe the population health paradigm and identify practice and payment models and strategies for population health management and creating a culture of wellness.
  • Examine the special microeconomic characteristics of the health care market.
  • Apply behavioral economic theory to the design of patient adherence strategies and provider incentive schemes which align with  special characteristics of the healthcare market. 
  • Explain the fundamental role and necessity for data management and analytics for population health, in care delivery, care coordination, patient engagement and organizational decision making.
  • Discuss the role of quality improvement in the context of population health management.
  • Describe ways in which a population health perspective affects healthcare priorities in the areas of transitions of care/care coordination, prevention, public health, and health policy.
